New York State Voter Registration Forms

All forms and applications must be mailed or hand delivered to the board of elections.

We are unable to accept any forms or applications by fax

Change of Party Enrollment

Forms RECEIVED at your local board of elections up to and including February 14th will be accepted and processed. If you submit your party change online through DMV the deadline is midnight February 13th for electronic delivery on the 14th.

Changes received on or after February 15th until seven days after the June primary will be set aside and opened the seventh day following the June primary and entered in the voter’s registration record.

The qualifications for voting in New York State are as follows:

  • Be a U. S. citizen
  • Be 18 years old (you may preregister at 16 or 17 but cannot vote until 18)
  • Not be in prison for a felony conviction
  • Not claim the right to vote elsewhere
  • Not found to be incompetent by a court

How to Register or Update Your Voter Record

Download a PDF copy and submit it at the Board of Elections Office or by mail to register or update any information that has changed.

You may also register or update online at the MYDMV website.

Voter Registration forms are available from:

  • U.S. Post Offices
  • Public Libraries
  • Village/Town Clerk Offices

You can also complete an Accessible version of the New York State Voter Registration Form on-line by clicking here, typing the necessary information and selecting the appropriate boxes.

Once the form is completed, you need to print the form and sign and date it. Then, mail or deliver to our office.
