The Children and Youth with Special Health Care Needs (CYSHCN) program seeks to improve the coordination of care for children and youth from age birth to 21 years who have special health care needs.
Families in the CYSHCN program have children who need extra health care and support. These children might have a serious or long-lasting physical, developmental, behavioral, or emotional condition.
The free CYSHCN program offers information, referrals, and advocacy support to help guide families by:
- Matching families with community resources and referring to community programs or local agencies
- Supporting families in navigating systems of care, for example accessing health insurance and finding a health care provider
- Connecting to peer supports
- Providing follow-up and short-term case management as necessary
- Identifying gaps and barriers to accessing care, resources, and services
- Acting as advocates with childcare providers, health care providers, and school districts
- Empowering parents and providers to be advocates
- Assisting in transitioning to adult services
