Birth Certificates In Person
Only the individuals listed on the birth certificate (parents or child), the legal representatives of the parents or child, or legal guardians with court ordered custody are entitled to request and receive certified copies of a birth certificate.
We accept walk-ins from 9:00am to 3:55pm, Monday through Friday. We do not make appointments.
Eligible applicants who arrive with a completed application, ID, and payment will be able to leave with their certificate, if the certificate is on file with our office.
To Order a Birth Certificate:
- Complete, print and sign a request form. Please use blue or black ink. Electronic signatures are not accepted. Forms are also available at our office.
- Review the Form: Please be sure to complete the certificate information and applicant information sections. Please list the child’s name and their mother’s maiden name – not married name. If both parents’ names are known, please include both parents’ names. Incomplete applications may result in processing delays.
- Payment: The fee is $30.00 for the first certified copy and $15.00 for each additional duplicate when ordered at the same time. We accept cash, checks and money orders made payable to Office of Vital Statistics, and credit/debit cards (Discover, Visa, and Mastercard only). If the cardholder or check writer is someone other than the applicant, the person making payment must also present ID.
- Identification: Please present identification when you make your request. Examples of acceptable identification include:
- Photo driver license with signature
- Non-driver photo license with signature
- Passport with signature
- Military ID with signature
- Employer’s photo ID with signature
- Photo social services card with signature
