What You’ll Do:
- Conduct nutrition assessments and create individualized care plans.
- Educate clients and caregivers on healthy eating and nutrition strategies.
- Collaborate with case managers and medical staff for coordinated care.
- Monitor progress and adjust nutrition strategies for long-term outcomes.
- Maintain accurate documentation and comply with program standards.
What You’ll Need:
- NYS Dietitian-Nutritionist certification (required).
- Bachelor’s or Master’s in nutrition, dietetics, or related field.
- Clinical or community nutrition experience preferred.
- Strong communication, organizational, and documentation skills.
- Commitment to quality service and equity in care.

What You’ll Do:
- Develop and monitor menus for home-delivered meals and senior dining sites.
- Conduct site visits to ensure food safety, quality, and compliance.
- Provide training and technical assistance to meal providers.
- Lead nutrition education and Evidence-Based Health Promotion programs (chronic disease, fall prevention, wellness).
- Collaborate with interdisciplinary teams and support interns/students.
What You’ll Need:
- Option A: Bachelor’s in dietary/nutrition technology, food service administration, or related field with ≥6 nutrition credit hours.
- Option B: Associate’s in Dietetic Technician program + NDTR/DTR registration.
- Valid driver’s license.
- Passion for wellness, food safety, and community-based nutrition.
