The Outreach and Navigation Team is part of the Adult System of Care. The liaisons help adults with mental health challenges find and connect to services like mental health care, SUD services, housing, or other important resources. The goal is to make sure they feel supported and stay connected to their communities and services to improve the quality of life.

Team members, called liaisons, work closely with hospitals, social service agencies, police, and other partners to help adults who are facing serious challenges. They focus on building trust and helping people stay engaged so they can improve their overall quality of life.

If you know someone 18 or older who could use this kind of help, you can refer them by emailing SOCOutreach@ongov.net and asking for a referral form. After you submit the form, the team will review it and follow up with you.
