If someone else will be paying, please order by <a href=\"https:\/\/onondaga.gov\/vital\/bmail\/\">mail<\/a> or <a href=\"https:\/\/onondaga.gov\/vital\/bfax\/\">fax<\/a>.<\/li>\n\n\n\n<li>We only offer domestic shipping for online orders. Please <a href=\"https:\/\/onondaga.gov\/vital\/contact\/\">contact us<\/a> if you need international shipping.<\/li>\n\n\n\n<li>Orders will be processed within 7-10 business days.<\/li>\n<\/ul>\n\n\n\n<p><\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>To Order a Birth Certificate:<\/strong><\/h3>\n\n\n\n<ol class=\"wp-block-list\">\n<li>Complete the\u00a0<a href=\"https:\/\/secure.ecitizenportal.com\/billpay\/portal\/ONGOV\/VS\" target=\"_blank\" rel=\"noreferrer noopener\">online request form<\/a><strong>\u00a0(secure server).<\/strong><\/li>\n\n\n\n<li>Review the Form: Please be sure to complete the applicant and mailing address portions. Please list the child&#8217;s name and their mother\u2019s&nbsp;<strong>maiden name<\/strong>&nbsp;\u2013 not married name. Please include both parents&#8217; names if both names are known. Incomplete applications may&nbsp;result in processing delays.<\/li>\n\n\n\n<li>Payment: Mastercard and Visa credit cards are accepted. The cardholder&#8217;s name must match the applicant&#8217;s name. The fee is $30.00 for the first certified copy and $15.00 for each additional duplicate when ordered at the same time.<\/li>\n\n\n\n<li>Cover Sheet: Once &#8220;Complete Order&#8221; button is clicked,&nbsp;a signature verification sheet will be e-mailed to you.&nbsp;Please sign and date the cover sheet then return it.<br><strong>Note:&nbsp;&nbsp;Every time you click &#8220;Complete Order&#8221; you card will be charged!<\/strong> If you are experiencing difficulties please contact our office at (315) 435-3241.<\/li>\n\n\n\n<li>Identification: We require identification which shows your photo, signature, and mailing address. Please make sure the copy is readable, large and clear. Examples of acceptable identification include:\n<ul class=\"wp-block-list\">\n<li>Photo driver license with signature<\/li>\n\n\n\n<li>Non-driver photo license with signature<\/li>\n\n\n\n<li>Passport with signature<\/li>\n\n\n\n<li>Military ID with signature<\/li>\n\n\n\n<li>Employer&#8217;s photo ID with signature<\/li>\n\n\n\n<li>Photo social services card with signature<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>If your ID does not show your current mailing address, we can accept two (2) of the following:\n<ul class=\"wp-block-list\">\n<li>Utility or phone bill from the past 6 months, showing your name and current address<\/li>\n\n\n\n<li>Mail from a government agency from the past 6 months, showing your name and current address<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>If you cannot provide any ID or utility bills showing your current address, or if you need us to mail the certificate to a third-party, please fill out the&nbsp;<a href=\"https:\/\/onondaga.gov\/vital\/wp-content\/uploads\/sites\/205\/2026\/06\/NotarizedPermissionForm.pdf\">Notarized Permission to Release Vital Records<\/a>&nbsp;form.\n<ul class=\"wp-block-list\">\n<li>This policy does not apply to law firms or other agencies requesting the record for legal or other official purposes.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>Verification: Your completed<strong>&nbsp;<\/strong>cover sheet&nbsp;and&nbsp;proper ID&nbsp;can be faxed to (315) 435-3614, or scanned and attached to an e-mail to&nbsp;<a href=\"mailto:vsorders@ongov.net\" target=\"_blank\" rel=\"noreferrer noopener\">vsorders@ongov.net<\/a>. If we do not receive verification within 5 business days, your order will be canceled. A certificate is only issued after a completed request form, signed cover sheet, proper identification, and payment are received.<\/li>\n<\/ol>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Expedited Orders<\/strong> (Domestic Shipping Only)<\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Priority Mail (USPS delivery 2-3 business days): Add $20.00 <\/li>\n\n\n\n<li>Next Business&nbsp;Day (UPS delivery): Add $25.00<\/li>\n\n\n\n<li>All other orders are sent by USPS First-Class Mail.
